Patient Portal - Luminello


I use Luminello to chart and maintain the health records of my patients and to manage my medical practice. The Luminello Electronic Health Record (EHR) is designed specifically for mental health providers and is fully compliant with all HIPAA regulations concerning patient privacy. The system also stores payment information and is PCI Complaint as well.

Current Patients

 

Current patients should use the Luminello Patient Portal to schedule appointments, check for messages from me, review documents and materials, update forms, etc.

New Patients

All new patients to my practice must first register on Luminello for an Initial Consultation. The registration process is free, easy and should only take a few moments to complete:

NEW PATIENT REGISTRATION
 
  1. Once on Luminello site, click Request a Consultation button located to the right of my photo and above my license information details.
     
  2. Carefully read the statement and proceed if you are not in an emergency situation by checking the box 'This request is not an emergency or urgent matter. I wish to proceed.'

    IMPORTANT: If you are in an emergency, please leave this website now, call 911 or go immediately to your nearest Emergency Room.
     
  3. Complete the brief registration form, click Next.
     
  4. Create your own Luminello Patient Portal login. Note your username and password and log in. 


Use these Tabs on the left side of your Luminello Home page:

  • Messages – You can send me secure email style messages attaching a file or document(s) if needed.
     
  • Demographic Info – Add your personal information and emergency contact details as well as details of your health insurance provider if you have one.
    Be sure to provide your current and active cell phone number.

    Provide the name, address and current phone number of your preferred pharmacy so that I may prescribe medications and to send them to your local pharmacy.
     
  • Billing - Add your insurance information including policy number, group number (if any), and any applicable deductibles and copays. Your method of payment for co-pays and deductibles is stored here are well.
     
  • Documents – Here is where you access a Questionnaire which you will need to complete before our first consultation.

    NOTE: I must have your Questionnaire answers at least 24 hours before your scheduled first appointment time in order for me to check your information and prepare fully for what might be needed during our initial discussion.

    Please read and sign the required policy forms. A document containing instructions on what to ask your health insurance representative is found here as well.

Once you have created your Luminello profile, the following forms and questionnaires to review and sign will be provided under Messages:
